Study title: USC Healthy Minds Research Volunteer Registry

You are invited to join the USC Healthy Minds Research Volunteer Registry. Your participation is voluntary. This document explains information about this registry. You should ask questions about anything that is unclear to you.

PURPOSE

The purpose of this registry is to maintain a list of volunteers who are interested in participating in research studies and manage recruitment and scheduling for studies. You are invited as a possible member because you are at least 18 years old.

PARTICIPANT INVOLVEMENT

To sign up for the registry, you will provide your full name, date of birth, gender, and contact information.  Some studies require a screening process, and additional information relevant to study eligibility (ex. MRI eligibility, ethnicity, race) may be added to your existing record in the registry.  The registry will also store information related to recruitment, scheduling, appointments, and completed tasks.  Please note, however, that results from or performance on study tasks will never be stored on the registry.

By joining the registry, you are only agreeing to let us contact you about research studies. When contacted about potential participation in a study, you will be provided details about the study and you will have the opportunity to ask questions before deciding whether or not to participate.

CONFIDENTIALITY

The members of the research team and the University of Southern California Institutional Review Board (IRB) may access the data. The IRB reviews and monitors research studies to protect the rights and welfare of research subjects.

Your information will be stored in Ripple™, a secure web application designed for storing and management of personally identifying information of research participants. Ripple infrastructure complies with the privacy and security guidelines of the Health Insurance Portability and Accountability Act (HIPAA), and information stored by Ripple is kept in a fully encrypted format inside dedicated databases that are segregated from other Ripple accounts, ensuring that only authorized study staff will have access to your information.

Furthermore, Ripple is used only for storing personally identifiable information of participants and is not used to capture other research data (e.g., questionnaires, health records, etc.). This ensures that the personally identifiable information and research data are segregated.  More information can be found at https://ripplescience.com/.

Your information will be kept confidential and your information will not be shared with anyone outside of the research team.  There is a small risk that people who are not connected with this study will learn your identity or your personal information.  Your information will be stored on Ripple indefinitely.

Your information will be used for research purposes only and will only be shared with other USC Healthy Minds researchers.
